Fitness Center Enrollment

An on-site fitness center is provided to all employees who wish to enroll in this voluntary benefit. If an employee elects to enroll, the following guidelines apply:

• If an employee chooses the Annual Enrollment program, $15.00 per month will be deducted from each paycheck.
• If an employee chooses the Month-to-Month program, $30.00 must be paid by check or cash by the first of each month.
